The Optimal Essentials dietitians have developed a questionnaire to assess your nutrition status to enable them to provide recommendations on how you may be able to improve your level of nutrition and general wellness.

The questionnaire has been developed based on the Mini Nutrition Assessment tool "MNA®".

The MNA® is an internationally recognised, validated nutrition screening and assessment tool that can identify patients aged 65 and above who are malnourished or at risk of malnutrition. The MNA® was developed nearly 20 years ago and is the most well validated nutrition screening tool for the elderly. Our dietitans use this tool to assess your nutrition status.

On completion of the questionnaire, we will send you the results  and our dietitians will provide you with their assessment of your nutrition status and recommendations of any of our products which may be able to assist you. If you have been approved for a Home Care Package you may be able to receive the full cost of the Optimal Essentials Products paid for under your Home Care Package directly by your Home Care Provider.
